She was born on August 14, 2019, daughter of Dominick Formato Jr. and Erica Formato.
A Graveside service will be held 12:00 p.m. Saturday, August 31, 2019 at Springdale Mennonite Church Cemetery by Pastor John Morrison.
Coffman Funeral Home and Crematory, 230 Frontier Drive, Staunton, is in charge of her arrangements.
